The intestines in humans are not just located there, but are fixed along the entire length by mesentery - sheets of the peritoneum - to the back wall of the abdominal cavity and organs. They do not fall out with a hose. It is generally quite difficult for the large intestine to fall out, they are inactive in comparison with the small intestine, which has a long mesentery.
It is the loops of the small intestine that tend to protrude into the wound when it is wide enough for the intestines to "fall out." But the mesentery does not allow the loops to completely fall out and get confused.
If the mesentery is damaged and the intestine is detached from it, then necrosis begins (it is in the mesentery of the vessels) and this part of the intestine will be removed.
