At the same time, diagnoses are repeated all the time - from year to year, from day to day, the doctor writes out the same words on paper, and this must be done very quickly. When I was an intern, I heard from an operating doctor of the highest category: "Medical histories should be written with one pen with a large distance between lines. Because the medical history is written for the prosecutor!"

As for illegibility, at least in my own area, and for all the doctors with whom I worked, I could read everything written. You understand the logic of the text, you know the terminology. Therefore, from the point of view of "encryption" it is meaningless)
